Incident investigation

This page contains a list of fatalities, serious injuries and serious dangerous occurrences that need to be notified to us by telephone.

When an incident or accident occurs, we receive notification in line with The Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR).

Details of improvement notices and prohibition notices served by our inspectors, and details of prosecutions in England and Wales taken by inspectors and the Procurator Fiscal in Scotland.
