Following an incident on 21 April 2013 when a road-rail vehicle (RRV) was involved in a 'runaway' while being on-tracked near Queen Street tunnel, Glasgow it ran down a gradient , struck a scaffold injuring a workman, before coming to rest in Queen Street Station. On 26 April, ORR (and RAIB) examined the machine involved, and another, subsequently finding that the model of RRV was not fitted with the effective engineering means to prevent an unbraked state occurring during on and off tracking. As there are a number of locations on the GB rail network where there is a sufficient gradient that RRV's could move uncontrolled or runaway, the Notice was served prohibiting the on and off tracking of these machines on such gradients, until suitable engineering controls were provided. 26 Apr 2013 26 Apr 2013 Complied

Following an incident on 8 March 2013 in which a road-rail vehicle (RRV) was involved in an incident at Hope Station when a track worker on their track renewals site was struck by a RRV and become trapped between the RRV and the station platform. Following an incident on 8 March 2013 in which a Quattro road-rail vehicle (RRV) was involved in an incident at Hope Station when a track worker was hit by the vehicle and become trapped between the RRV and the station platform a Prohibition Notice was served on Quattro Plant Limited as the RRV needed to be taken out of service for examination.
