Following an incident in March 2012 where a Network Rail contractor received an electric shock from a signal location cabinet, an Improvement Notice was served on Network Rail which required them to review their 650V signalling distribution equipment. The results of this review showed that significant numbers of cabinets in public places were not earthed or bonded – which was contrary to the assurances given by Network Rail. This IN requires NR to perform a review of all cabinets in public places, and to ensure that earthing and bonding is installed in all of them. Additionally, it requires NR to introduce a maintenance process to ensure that all cabinets remain in a safe condition.19 Nov 201321 Feb 2014Open

On 30 April 2013 at Broomloan Depot in Glasgow a locomotive and wagon operated by SPT became detached from each other and during the recovery operations the unattached wagon ran down the ramp from the depot into the tunnel of the inner circle of the subway system.
The notice was issued due to SPT's failure to demonstrate a suitable and sufficient assessment of the braking and coupling requirements and maintenance of their engineering vehicles, thereby placing their employees and contractors at risk from uncontrolled movements of such vehicles.

As a consequence a Network Rail employee amputated part of his thumb between the moving switch blade and stock rail whilst undertaking welding inspections at Bristol Junction on 772B points on the 23rd October 2012.
